Overview

Currently when patients have to continue their care post hospital at an extended care facility such as skilled nursing rehabilitation LTAC and other types they are provided with minimal information and it is up to their families to find the facility to go to. It is a very frustrating and inefficient process for all participants patients families hospitals and facilities.
