Today when a patient is discharged from a hospital or skilled nursing facility to post acute care such as assisted living homecare board and care RCFE (to just name a few) or needs rehab care durable medical equipment (DME) or even transportation services the discharge planner patient caretaker and family rely on antiquated materials and hours of phone calls to locate the provider which fits their exact needs. OpenPlacements web application serves patients and their caregivers by helping them search and qualify care providers that best meet their specific needs. OpenPlacement then connects the patient caretaker family and/or care professional directly with the care provider. OpenPlacement gives care providers direct access to new referral sources at hospitals and with patients who otherwise would never know about them leading to increased business and streamlining the process of patient intake.
